From the Helpdesk Perspective

Although providers can obtain verbal consent for the Telehealth Addendum, written consent must be obtained for new patients and for expired consents. This tip sheet is designed to help you understand the process the caregiver completes.

Electronically signing consents 

  1. When families receive a consent packet, they’ll get an email from DocuSign stating “Sender’s Name sent you a document to review and sign,” with the sender’s email address included. To start the signing process, they click the yellow “review document” button.

  1. After opening the form, caregivers will first need to select the “Continue” button to begin the signing process. Thereafter, caregivers will be prompted to complete each required section after reading the documents. Some parts are required for caregivers to complete the form.

  1. The gray boxes are optional fields. Both the yellow signature sections and the red boxes are required fields. The caregiver cannot “Finish” the form until all required items are filled out.

 

  1. If they cannot navigate signing electronically, they can opt to “Print & Sign” the document by clicking on “Other Actions.” After they’ve completed the form by hand, they can scan it in and email it to the provided address or fax it to the provided number.

 

 

 

 

 

 

  1. Patients/Parents have the most trouble with uploading a copy of their driver’s license and insurance card or Military ID. There are 4 paperclip attachment icons. Red and yellow means there is nothing attached. Grey means there is an attachment. You want to make sure that ALL 4 paper clips turn grey. A good question to ask is “How many paper clips are Gray or Yellow?” 

 

  1. Click the red and yellow paperclip.

 

  1. You will see 3 options “Photo Library, take a Photo or video, and Browse.” The best option is to take a photo. Have them Click take a photo, verify that they took the photo and have them press “Done.” (Repeat this 4 times for all for attachments). (Front of ID, Back of ID, Front of insurance card, and back of insurance card).

 

  1. Once all the required fields are filled out and they hit the “Finish” button, they are done. They will be notified if they missed a section. 
  2. Once complete, the document is then sent back to the sender via DocuSign to add his/her signature witnessing the forms.
  3. After the sender signs, a copy automatically goes to the caregiver, the sender, and Health Information Management to upload into the patient’s chart.

Please note that we have different packets created for different purposes. New patients will receive a consent packet with:

  1. General Consent
  2. Financial Consent
  3. Telehealth Addendum
  4. Information on where to find Patient’s Bill of Rights, Privacy Practices, etc.
  5. Place to upload ID/Insurance Card

Annual consent packets only contain:

  1. General Consent
  2. Financial Consent
