MyChart Sign-up (grant Proxy Access)
Proxy access allows parents to use the patient portal to view/access their child's information. This guide allows you to sign-up a parent for MyChart. This can be achieved by sending a MyChart link from Epic to the parent’s email.
Sign-up or give proxy access to a parent
- Navigate to the MyChart Administration activity.
- On the bottom left of patient lookup, click Switch to patient Lookup.
- Search for the patient’s name and DOB, and press enter.
- Double click on patient name or click Accept.
- If it is a parent or guarding calling in, confirm their identity by asking their name and DOB. Click Show Potential Proxies.

- Click Grant Proxy Access.
This section includes emergency contacts and guarantors for the patient. |
- Click Search MyChart Accounts to see if there is already an account or an initiation.
- Confirm that the DOB is the same. In the form that appears, search for the person you want to add by name, MyChart username, email, or Social Security number.
- If the person you want to add is listed under the Suggested Contacts section, select the name, and click Accept this Account.
- To initiate a new MyChart account, verify their name and DOB matches and click Create Non-Patient Account.

- Whether you selected a patient proxy or a non-patient proxy, you will be brought to a screen to edit the relationship between the patient and proxy. Choose the proper Relationship and click Accept.
Patient's biological parent: The relationship type would be Parent Accessing Child. Someone other than a biological parent, transfer the call to HIM. |
- You will then be presented with activation options. If the user already has an active MyChart account, they will gain immediate access.
- Click Send Email to send an immediate email.
The proxy is now listed in the People Who Can Access the patient’s chart. |
